What the sign-up form collects and how to stop hearing from us.

What the form collects

Three things: your name, your work email and the name of your practice or facility. Nothing else is asked.

How we use it

We use your email only to tell you when Settlane Health opens. We do not sell or share it. The form is run by our form provider, Jotform, which stores your entry for us.

Stopping the notes

Every email we send carries a way to unsubscribe. You can also write to Operations@settlanehealth.com and ask to be removed.

Patient information

Please do not send patient names or other patient details through this site or by email. The sign-up form never asks for them.
